Tray Safety Report

1 in 6 trays didn’t match its ticket

Tropical Trees Skilled Nursing, Testville, MD
Jul 22 – Sep 4, 2026Prepared Sep 5, 2026

Every tray, every shift, photographed at the line. Between Jul 22 and Sep 4, roughly one tray in six at Tropical Trees Skilled Nursing did not match its ticket and was held at the scan point. Here are ten of them.

What the 854 intercepts were

Five things worth coaching on

  • Critical scoring and allergen naming worked: all 10 cases scored 2–3 and named the critical issue; 9 said “Hold tray.”
  • Modified-texture failures recur: 8 of 10 show wrong food form (whole banana on puree repeats; 5 mechanical-soft/chopped/ground cases Aug 18–Sep 4).
  • One source order is unsafe on its own: Case 03 pairs a puree diet with a whole-baked-potato standing order. Order-template/master-data fix, not just plating.
  • Close the loop: the record shows every hold; logging the re-scan proves the fix and turns a catch into a closed case.
  • Semantics + vision prevented false escalation: ticket dislikes weren’t promoted to allergy errors.

Ten of the 854

Sample trays from the intercepted set. Each photo shows the ticket and the plate that contradicted it.

Egg served against a documented egg allergy

AllergenScore 3 · critical
Visible evidence
Ticket clearly says Allergies: EGGS; two unmistakable hard-boiled egg halves are on the tray.
Live handling
The system flagged the tray and named the allergen directly: “Hard-boiled eggs detected (potential allergen).” It also noted missing yogurt and butter.
Why it matters
Egg is a major allergen. The detection worked as intended and the hold is on the record; the re-scan after correction is what closes the case.

Yogurt served against dairy and milk allergies

AllergenScore 2 · critical
Visible evidence
Ticket lists DAIRY and MILK allergies; a sealed Dannon Light + Fit Greek yogurt is visible.
Live handling
The explanation identified the yogurt conflict and explicitly said “Hold tray.”
Why it matters
Milk is a major allergen, and allergic reactions can be severe or life-threatening.

Whole potato and intact entrée component on a puree order

PureeScore 2 · critical
Visible evidence
Ticket says Diet Order: Puree; a whole baked potato and an intact non-pureed entrée component are visible. Standing-order text also calls for a whole baked potato.
Live handling
The explanation identified both non-pureed foods and said “Hold tray.”
Why it matters
The source instructions are internally contradictory, creating a repeatable system-level hazard.

Whole banana on a puree diet

PureeScore 2 · critical
Visible evidence
Ticket says Puree; a whole, unpeeled banana is beside the pureed entrée.
Live handling
The explanation identified the banana and said “Hold tray.”
Why it matters
A whole banana visibly does not meet puree appearance/form requirements and requires chewing. Severity depends on why the puree diet was ordered.

Repeated whole-banana puree failure

PureeScore 2 · critical
Visible evidence
Ticket says Puree, No Added Salt; a whole, unpeeled banana is visible.
Live handling
The explanation identified the whole banana and said “Hold tray.”
Why it matters
The same high-consequence presentation recurred three weeks after the earlier banana case, suggesting a process pattern rather than an isolated event.

Chicken strips on a ground-meats order

Modified textureScore 2 · critical
Visible evidence
Ticket says Mech Soft / Grd Meats; chicken is served in long grilled strips, not ground, with chunky vegetables.
Live handling
The explanation identified the intact chicken strips and said “Hold tray.”
Why it matters
The visible meat form directly conflicts with “ground meats.” Potential harm depends on the resident’s chewing/swallowing impairment.

Multiple intact foods on a chopped-meats order

Modified textureScore 2 · critical
Visible evidence
Ticket says Mech Soft / chopped meats; long chicken strips, raw leafy salad/onion, and a whole breadstick are visible.
Live handling
The explanation identified intact chicken and breadstick and said “Hold tray.”
Why it matters
This tray and the chicken-strips tray above reached the scan point 11 minutes apart, pointing to a shift-level control issue worth reviewing.

Whole chicken tenders on a chopped order

Modified textureScore 2 · critical
Visible evidence
Ticket says Mech Soft / chopped; whole breaded chicken tenders are visibly unchopped.
Live handling
The explanation said “Hold tray for re-prep.”
Why it matters
The meat form is plainly inconsistent with the ticket; severity depends on the clinical indication for modification.

Intact sandwich and whole banana on a chopped order

Modified textureScore 2 · critical
Visible evidence
Ticket says Mech Soft / chopped; an intact English-muffin egg-and-cheese sandwich and a whole banana are visible.
Live handling
The explanation identified both foods and said “Hold tray.”
Why it matters
This latest-window recurrence shows modified-texture risk remained active through the end of the review period.

Intact burger despite explicit cut-food direction

Modified textureScore 2 · critical
Visible evidence
Ticket says Mech Soft / chopped meats and directs staff to cut food on the plate; an intact burger/whole bun and large vegetables are visible.
Live handling
The explanation said “Hold tray for correction.”
Why it matters
The tray visibly bypassed both the diet-form instruction and the explicit cut-food direction.

Worth a second look

Four close calls a photo alone cannot settle. Check these on the floor.

Mechanical-soft pizza presentation

Close callScore 2 · critical
Why it is close
Ticket says Mech Soft; large pizza slices with intact crust and large vegetable pieces are visible. A photograph cannot prove softness.
Human resolution
Review the original disposition. Recreate the menu item under comparable conditions and apply the facility-approved texture test at service temperature—and IDDSI only if prescribed.

Modified meat in a whole bun with cucumber

Close callScore 2 · critical
Why it is close
Ticket says Mech Soft / Grd Meats; chopped hot dog is served in a whole bun with cucumber. Meat was modified, but bread and side may remain problematic.
Human resolution
Review disposition and local bread/side standard. Recreate the item under comparable conditions and test at service temperature.

Whole banana and tater tots on mechanical soft

Close callScore 2 · critical
Why it is close
Ticket says Mech Soft / chopped meats; whole banana and tater tots are visible. Both may be soft enough, and no meat requiring chopping is shown.
Human resolution
Review whether the order modifies all food or meat only. Recreate and test both foods at service temperature under the prescribed framework.

Yogurt with an obscured allergy field

High consequence · unconfirmedScore 9 · pass
Why it is close
A sealed yogurt is visible. Readable ticket text says Dislikes: Milk, while the allergy field is obscured; the photo cannot establish an active milk allergy.
Human resolution
Check the authoritative allergy record and unobscured source ticket. If milk allergy was active, escalate and review disposition.
